Long Beach, CA — A 48-year-old woman was killed during an apparent elevator malfunction Wednesday, December 7, 2011, at Cal State Long Beach’s Foundation building, reported the OC Register.

According to reports, Annette Lujan, of Huntington Beach, tried to exit the elevator after an apparent malfunction that occurred between floors. As she attempted to crawl out, the elevator shifted down and crushed her.

Long Beach Fire Department’s Urban Search and Rescue team arrived at the scene and worked for about an hour and 20 minutes to back up and lift the elevator off of Lujan, by using a pulley system.

School officials revealed that Lujan was a nine-year employee of the university’s grants and contracts office.

Cal-OSHA is currently investigating the fatal elevator accident. The cause of the malfunction is currently unknown at this time.
