Milwaukee, WI — An industrial accident at northwest side business left a 58-year-old man dead Tuesday morning, August 30, 2011. The fatal industrial accident occurred at ASI Technologies, located on the 5800 block of North 95th Court at around 5:20 a.m., reported Today’s TMJ.

According to Milwaukee Fire Department officials, the victim who was identified as Henry LaFlore, sustained fatal head trauma when his head was crushed in a press at the business.

Responding emergency crews pronounced LaFlore dead at the scene of the workplace accident. Members of the structural collapse unit were called in to lift the machine off the victim.

Preliminary reports indicated that a machine and cable wire broke, which caused the press to crush LaFlore. Investigations are still trying to uncover what caused the cable wire to snap.

LaFlore reportedly has been an employee at ASI Technologies for at least 30 years.

Investigations into the fatal industrial accident are underway. The U.S. Occupational Safety and Health Administration (OSHA) are also expected to report to the scene to conduct its own investigation to determine if any workplace safety violations took place.
