Utah Industrial Accident: Roosevelt man killed in oil field accident

Duchesne County, UT—An apparent industrial accident at an oil field killed a Roosevelt man after falling into the pump jack. The fatal work-related accident occurred sometime between 12:30 a.m. and 7 a.m. Sunday, January 30, 201 at a location in the Wells Draw area of Nine Mile Canyon, as reported by the Desert News.

Accoding to the Duchesne County Sheriff’s Department, Cody Earl McMullin, 32, was reportedly removing water from a pit at the well site, when he decided to climb onto the pump jack for an unknown reason.

McMullin fell into the pump jack, where he died of crushing injuries. An oil-field worker found McMullin’s body when they arrived at the well site on Sunday morning.

A full investigation into the fatal industrial accident is underway. Officials with the U.S. Occupational Safety and Health Administration (OSHA) are expected to arrive at the scene to conduct an investigation.
