Henryville, IN—A motor vehicle collision (MVC) involving a minivan and 18-wheeler claimed the life of a Columbus man around 5:30 a.m. Thursday, June 17, 2010. The fatal accident occurred in the northbound lanes of Interstate 65 between Henryville and Scottsburg, according to information provided by Courier-Journal.com.

Reports indicated 31-year-old Kristina Forester was driving a minivan north on I-65 when she attempted to change lanes. Upon doing so, her minivan slammed into a tractor-trailer for unknown reasons.

There were four people, including Forester, occupying the minivan at the time. An adult passenger in the minivan, 40-year-old Robert J. Kellen, succumbed to his traumatic injuries at the scene.

Forester and her two children, whose ages and names remained undisclosed, were transported to Kosair Children’s Hospital via ambulance. They were treated by doctors and nurses and subsequently released.

The driver of the semi-truck, 38-year-old Andrew Richter, escaped injury from the two-vehicle wreck.

According to Indiana State Police (ISP) Lt. Jim Hickerson of the Sellersburg post, fatigue may have contributed to the crash, given that it occurred early in the morning.

Investigations into the deadly van-rig accident are expected to be underway.
