Maybrook, N.Y.—A wall collapse at an upstate New York construction site left one person dead and two others injured on Monday, December 2, 2013. The fatal accident occurred around 12:30 p.m. at 918 Homestead Avenue in Orange County, the New York Daily News reported.

According to reports, the retaining wall collapsed as concrete was being poured.

Responding emergency medical crews had to lift the wall off the body of one worker, who was ultimately pronounced dead.

A second injured worker was airlifted to Westchester Medical Center for treatment of multiple injuries. A third worker was transported by ambulance to Orange Regional Medical Center for treatment.

“Our thoughts and prayers are with the workers involved in today’s accident as well as their families and loved ones,” DEP spokesman Adam Bosch said in a statement. “OSHA will lead a thorough investigation into the circumstances that led to the accident.”
