Phoenix, AZ—A forklift accident at a Phoenix recycling facility left a worker dead on Wednesday afternoon, September 22, 2010. Phoenix fire and police officials were called to the Hendrix Recycling Inc. near Broadway Road and Ninth Street about 2:30 p.m., after receiving a report of an accident, as reported by AZCentral.

According to authorities, the worker was operating a forklift in the backyard area of the recycling facility, when it collided with a larger piece of equipment. The impact caused the forklift to overturn, throwing the operator from the forklift. The forklift then rolled onto the worker and killed him.

Officials believe the larger piece of equipment in which the forklift hit, is a front loader, but it is unknown if it was being operated at the time of the accident.

Other employees desperately tried to lift the forklift off the worker and pull him out from underneath. The victim’s injuries were reportedly too severe.

Responding emergency crews pronounced the man dead at the scene.

A full investigation is underway. Officials with the Occupational Safety and Health Administration will also be conducting an investigation.
